The Health Promotion Case Management (HPCM) program uses a holistic approach to health and wellness. This approach appreciates that to prevent sexually transmitted and blood borne infections, we must also address the impacts of structural violence. HPCMs help to address the social determinants of health.
The program promotes health & wellness in all aspects of one's life, including the physical, social, cultural, spiritual and mental realms. Our case managers provide a broad spectrum of care in collaboration with diverse communities.
Our team of case managers prioritizes the wellness of the following communities:
- Gay and bisexual men
- Men who have sex with men
- Women (cis & trans)
- People who use drugs
- Sex workers
- Indigenous people
- Newcomers and refugees
- Trans, non-binary and gender-diverse people
- HIV-negative people who are having sex with people living with HIV who want support with HIV prevention
